Equity members in good standing are automatically enrolled for the upcoming calendar year each November.

An Equity Dental Network access card will be mailed to each Equity member in good standing after payment of basic dues in November.

Your spouse (or spousal equivalent) plus dependent children under age 25 are eligible to be enrolled. You may enrol dependents online here. 

An enrolment form is also available for download here or upon request (by mail). Downloaded forms must be returned by mail (to 200 – 5700 Yonge Street, Toronto, ON M2M 4K2).
