Dental services must be provided by dentists within the Equity Dental Network. You may select any Network dentist convenient to you or your family members. You can find a dentist participating in the Network here and you can enrol your dependents here.

Simply show your Equity Dental Network access card to receive a direct 25% discount on services performed by the general dentist and dental staff.

No claim forms are required. There is no waiting for reimbursement from the plan. There are no annual maximums.

Coverage is in effect from January 1 through to December 31 each year.
