Future Focus Dental Plans Ltd. administers the Equity Dental Network on behalf of its member groups. We maintain the highest standards of integrity in the operation of our business. In the course of our business, it is necessary to collect, record, store, process, transmit and disclose personal information about individuals. All such activities are done in a manner that we believe to be consistent with both generally accepted privacy standards under the PIPEDA legislation, as well as ethical business practices.

Purpose
Personal information is collected, used and disclosed for the purposes of member identification, billing and communication in regard to their receiving dental benefits for themselves or their dependents.

Consent
Under the terms of your group’s Dental Benefits Agreement or your Individual Membership Agreement, your provision of your personal information to your group and/or Future Focus Dental Plans or Equity Dental Network, and/or your claiming of dental benefits under your membership shall acknowledge your consent to our using your personal information for these purposes. You may withdraw your consent at any time in writing, and if you wish to do so, we will explain the implications of your doing so. If we intend to use that personal information in the future for a different purpose, we will obtain your consent for doing so.

Disclosure and Limits
Only required information for the purposes identified will be collected. Personal information will only be disclosed:
a. As required in the normal operation of our business;
b. Where required by law or a court of proper jurisdiction;
c. To protect the interests of Future Focus Dental Plans, Equity dental Network, or its associated companies, officers and employees against misrepresentation, fraud or other illegalities, prosecutorial actions or other unwarranted proceedings.

Retention
Personal information will be retained only as long as needed for the intended purposes or as required by law. The personal information is stored in secure databases, and in locked offices during non-operating hours.

Access
It is your right to know the personal information maintained about you. Written requests by you to know more about the personal information we keep will be responded to in a reasonable period of time at no cost to you. Requests for copies of personal information records to be delivered to you will incur a handling fee that will be billed to you in advance of the reproduction and delivery of the personal information to you. You have the right to request the correction of any inaccurate information in your file by contacting our Privacy Officer at privacy@futurefocusonline.com.
